Ok for this we need to customize some code in a call-to-action file so please follow the below steps.

1. Go to theme folder consultstreet-pro >> template-parts >> index-cta.php file.

2. Open this file and paste the link code in this file https://pastebin.com/whS15sX6 and save the file.

3. Add the contact form shortcode in the Button Text field.
